United Health Care (UHC) patients must fill out special forms before their initial visit for physical therapy.  Listed below are the links to those forms.

UHC patients must print out and bring a completed UHC Patient Health Questionnaire form to their initial appointment.

United Health Care patients must also chose the form(s) that pertain to their specific condition or injury.

1.  Neck Injury Form
2.  Lower Back Injury Form
3.  Shoulder, Hand or Arm Injury Form
4.  Lower Extremity Injury Form

United Healthcare patients should also click on the Axiom Patient Introduction forms link below and use your print function to print the forms. Please fill them out, and bring them in on your initial appointment with the appropriate United Healthcare forms.
