Thank you for choosing Atlanta Falcons Physical Therapy as the provider for your physical therapy or occupational therapy program.  Before your initial evaluation you will need to fill out our intake forms.  You may download them from our New Patient Forms page or fill them out when you come in for your initial visit.

IIf you need directions our physical therapy clinics, go to the Locations page and pull up directions to the facility at which you will be receiving your therapy.

We will verify your insurance before your initial evaluation, explain your therapy benefits, and assist you during the reimbursement process. Even though we verify your insurance this is not a guarantee of payment so please make sure you review your insurance policy.

We accept Medicare, workers' compensation, auto accident medical coverage(med pay), and attorney liens.  Please review our Insurance Companies page to see if we accept your insurance plan.  Atlanta Falcons Physical Therapy accepts many other insurance plans than those listed.  Please call (404) 367-2200 if your plan is not listed.
